Electrolyte imbalance

Major electrolyte present in body include (
Na-K-Cl-Ca-P-HCo3 )

:- *Importance of electrolytes

1. Play important role in maintaining of osmotic
P.

The major extra
cellular Na

The major
intracellular K

2. Play important role in regulating of
body fluid

3. Regulate he acid base of the body

Bicarbonate:
Buffer system of the body (regulate PH 7.35-7.45 "base", dissolved Co2 {H2Co3} "acid")

4. Play important role in neuromuscular
transmittion

5. Activation of some chemicals as
enzymes as: Phosphodehydrogenase E

Na

1. Extra cellular electrolyte

2. Play important role in regulating
osmotic P. of extra osmotic P. under control of Aldosterone H

Disturbance in Na:-

Hyponatremia

decrease of Na from E.C.F less than 135-145 ME q L

Etiology:

-decrease
Na intake & increase water intake

-uses of fluid therapy without Na

-increase Na loss from the intestine in acute

diarrhea & enterotoxaemia (E.Coli )

-increase Na loss from the
skin in excessive sweating & burns

Pathogenesis:

decrease Na from E.C.F;decrease
osmotic P., excessive secretion of water through urine & Dehydration
decrease blood volume & peripheral circulatory failure & anaerobic
oxidation & lactic acid production (Acidosis)

*Classification of Dehydration acc. to Na content:

1.
Hypertonic dehydration

Loss of water > loss of Na in
watery diarrhea

2.
Hypotonic dehydration

Loss of Na > loss of water in enterotoxemic E.Coli

3.
Isotonic dehydration

Water loss = Na loss

Clinical signs of hyponatremia (weakness, depression, anorexia)

Detected by: estimation of Na balance

Hypernatremia

increase Na intake in diet & decrease
water intake in case of hypertonic dehydration, deprivation of water from
animal, animal unable to take water as in esophageal obstruction

K

·
The major intracellular electrolyte

·
Play role in maintaining intra osmotic P.
(I.O.P)

Hypokalemia

Causes:

1-Decrease K intake
& increase K loss in metabolic
alkalosis & increase K loss from kidney
by using diuretics or due to decrease H ions

2-Abomasal diseases (torsion & impaction & displacement)

3-Intestinal obstruction at 1st portion

4-Excessive use of diuretics:

increase secretion of H&
Cl lead to increase K loss

reabsorption of Na & HCo3

Clinical signs

·
-muscular
weakness

·
-cardiac arrhythmia

Hyperkalemia

increase K level in blood more than 4-6 MEq

Causes:

1.
Metabolic acidosis ( loss of HCo3 & H
conc. Lead to increase K )

2.
Enteritis

3.
Late stage of diarrhea

Clinical signs

-cardiac arrhythmia

-dullness,
depression, anorexia

Cl

Hypochloremia

Causes:

Diseases
of abomasum or 1st portion of intestine
-

Excessive
use of diuretics
-

Acid Base Balance

Def:-

Regulation of PH of blood between 7.35-7.45

This will be achieved by:
action of HCo3 & dissolved Co2 ( carbonic acid )

-Decrease HCo3 & Increase H2Co3>>>Acidosis

-Decrease H2Co3 & Increase HCo3 >>>Alkalosis

Acidosis

When PH of blood less than 7.3

Due to: -Decrease HCo3 in blood or

-Increase dissolved O2 in
blood

Types:

*Metabolic
acidosis: due to decrease HCo3 level in blood

*Respiratory
acidosis: due to increase dissolved Co2 level in blood

Metabolic acidosis due to:

1.
Loss of HCo3: in early stage of diarrhea

2.
Excessive accumulation of acid: in lactic
acid production due to impaction ( Cho engorgement ) or prephiral circulatory
failure

3.
Combination of both: in late stage of
diarrhea

Respiratory acidosis due to:

Increase dissolved Co2 in blood & in lesions in lung as pneumonia, hydrothorax and chronic alveolar
emphysema

Compensatory :-

Metabolic acidosiss loss of acid through
urine acidic urine

Respiratory acidosis loss of HCO3 during respiration by
increase respiratory rate (hyper ventilation)

Clinical signs of
acidosis

1. Dullness ,
depression

2. Increase
respiratory rate (hyper ventilation)

3. Increase heart
rate

Muscular weakness

5. In sever
condition comma and death

Compensation

·
Hyperventilation (loss of co2)

·
Movement of k from intracellular fluid to extra
cellular fluid (hyperkalemia)

Treatment

In addition to treatment of dehydration , you should
put alkalnizing agent or antiacid (NAHCO3)

Alkalosis

Increase of blood ph

The ph of blood become <
7.45

Metabolic alkalosis increase of the
bicarbonate content in the blood

Respiratory alkalosis decrease dissolved CO2
in the blood

Etiology

Metabolic
alkalosis

1. Excessive loss
in acid as in:

Abomasal diseases
(tortion-impaction-dilatation-gastric infection in monogastric
animal-gastritis)

Excessive vomiting

2. Excessive use of
alkalnizing agent in treatment of acidosis

Respiratory
alkalosis

Excessive breathing which accompanied with excessive exercise excessive loss of CO2
via breathing
respiratory alkalosis

Clinical
manifestation of alkalosis

3. Hypoventilation

4. Cardiac arythmia
(hyperkalemia)

5. Muscular tetany

6. Dullness ,
depression

Treatment

Using acidifying agent as
kcl amm. Cl nacl (saline)

N.B

The normal saline used only in
treatment of metabolic alkalosis ( in mild , moderate cases)

In sever cases : combination
between normal saline and kcl give good result

Dehydration

Mainly accompanied with
acidosis but may accompniied with alkalosis in case of

Infection of abomasum

Loss of water from body

Loss of acid
"alkalosis"

Main causes of dehydration

Acidosis
alkalosis

Acute diarrhea

Loss of water
loss of electrolyte

From intra&extracellular
fluid
from blood

Shrunken appearance
haemoconcentration

Sunken eye
decrease blood flow to kidney "oligurea"

Prephiral
circulatory failure acidosis

Loss of electrolyte

Na cl hco3 k

Hyponatremia hypochloremia acidosis hypokalemia

Compensatory mechanism

Hyperkalemia
hyperventilation

Due to acidosis which result
from loss of bicarbonate

Treatment of elecrolytr
inbalance

1.
We use NaCl – NaHCO3

2.
Treatment of

·
Hypo ventilation

·
Cardiac arythmia

·
Muscular tetany

3.Fluid
therapy without k + normal saline (nacl)

Abomasal tortion or gastritis excessive production of hcl inside
abomasums

Decrease hcl from blood

In case of obstruction trapping of Hcl
inside abomasum
decrease H , Cl from blood

Decrease H

Excessive loss of K through
renal tubules

So in abomasal infection

·
Hypochlormic alkalosis

·
Hypokalemic alkalosis

Decrease Cl

Increase reabsorption of HCO3
from renal tubules
